Before your first appointment
Before your appointment you will need:
1. A referral from your medical provider for an Occupational Therapy Driving Evaluation.
2. A valid driver's license or permit

What happens at my first appointment?
We begin with a clinical evaluation based on your medical history and your driving goals. The evaluation may include a test of your thinking, visual, and/or physical skills for driving. Please be prepared to give a complete medical history, including any current medications.
If indicated, we proceed to an on-road evaluation in a driving rehabilitation vehicle equipped with an instructor’s brake.
At the conclusion, we will provide you with recommendations based on your goals, fitness to drive, adaptive equipment and/or training needs.

Appointment Cancellation and No Show Policy
Effective January 1, 2022
When you schedule an appointment with Smith OT and Driving we set aside enough time to give you the highest quality services. If you need to cancel or reschedule your appointment please contact your therapist or the office as soon as possible, and no later than 48 hours prior to your appointment. The following is our appointment cancellation/no show policy:
- Evaluation: When we schedule your evaluation (a three-hour appointment) we take a deposit of $100.00. If you do not show up for your evaluation or cancel less than 48 hours before your appointment, we retain your deposit.
- Training: If you do not show up for your training session or cancel less than 48 hours before your appointment, you will be charged for a one-hour training session.
We understand there may be times when you have an unforeseen emergency. If there are extenuating circumstances:
- Please contact your therapist via phone as soon as possible
- If your therapist does not answer the phone, call our main office at 707-546-9160. The office staff will answer the phone as Redwood Pediatric Therapy Associates. This is the parent company of Smith OT and Driving.
