There are two documents needed for the driving evaluation to occur:

A prescription from your doctor for a driving evaluation.

A valid license or permit.
- If your license has been withdrawn or revoked for any reason, I can help you obtain a special instruction permit. This permit allows you to drive within particular restrictions and will enable you to complete the on-road portion of the driving assessment (and follow-up instruction if needed).
Medical History
Be prepared to give a complete medical history, including current medications.
Cost
The cost of the clinical portion of the driving evaluation is $120, and the on-road portion is $380.
